ER Discharge Coordinator

Job Summary

As an ER Discharge Coordinator at TriStar Horizon Medical Center, you will be responsible for Point of Care (PoC) Patient Bedside scheduling as part of the company’s centralized ER and Urgent Care Follow-up programs. The role involves creating and maintaining a positive customer experience in a patient-centered, quality-driven environment.

Qualifications

• Speaks with customers, patients and stakeholders in a pleasant and professional manner
• Communicates appropriately and clearly with department leadership, co-workers and customers/patients
• Schedules patients for follow-up care bedside when appropriate
• Ensures performance and quality standards are met
• Provides superior customer service to internal and external customers and patients
• Suggests ideas for positive changes to department policies and procedures
