As an Engagement Care coordinator you will be primarily responsible for assisting in the immediate care coordination needs of youth hospitalized due to a behavioral health condition that are enrolled in the OhioRISE program. This position will support families with the hospital discharge process, educating families and community partners about the OhioRISE program, and making referrals for follow up care and services.
