About This Role

This is a great opportunity for an individual passionate about serving the underserved community in Los Angeles County. This role helps bring the PACE (Program of All-Inclusive Care for the Elderly) program to frail seniors, assisting them with Medicaid or Medicare applications to enable them to live at home as long as they wish. The ideal candidate brings direct experience with the Medicaid and Medicare new/renewal process.

Responsibilities

  • Initiate, assist, and manage Medi-Cal applications for prospective participants, ensuring timely and accurate completion.
  • Conduct home visits to complete the Medi-Cal application process.
  • Assist participants, families, and caregivers in completing all necessary paperwork for Medicaid re-determination or new applications.
  • Maintain documentation and record all enrollments in CRM software, ensuring accuracy and compliance.
  • Strictly adhere to DHCS and CMS regulations and policies regarding marketing practices for the PACE Program.
  • Collaborate with the Finance Department to prepare reports for data analysis and validate Medicaid payments.
  • Monitor and maintain participants’ Medicaid eligibility (recertification of Medi-Cal).
  • Support the Enrollment and Outreach Specialist as needed and provide community resources to prospective participants or families/caregivers.
  • Collaborate with or contact county/state government specialists to ensure submission of required documentation or obtain additional information.
  • Contribute to initiatives exploring AI and its potential to transform operations and member services.
  • Perform all other duties as assigned.

Qualifications & Experience

  • Knowledge of the California Medi-Cal System and Social Security process.
  • 2-4 years of relevant experience.
  • Experience working with State/County eligibility workers.
  • Experience working with frail elderly populations.
  • Bilingual skills in Spanish, Cantonese, Mandarin, or Vietnamese preferred.
