Position at a healthcare facility providing direct observation and companionship to patients who require continuous supervision to ensure safety and well-being. Works closely with nursing and medical staff to monitor patient condition, prevent harm, and assist with non-medical tasks.
Responsibilities
- Monitor patient condition and vitals, working directly with the RN to provide necessary care.
- Provide constant 1:1, direct supervision of assigned patients to ensure safety.
- Prevent patients from self-harm, falls, or other injuries by maintaining vigilant presence; notify RN of any signs or plans of self-harm.
- Report changes in patient behavior, condition, or needs to nursing staff promptly.
- Assist patients with basic needs such as eating, drinking, personal hygiene, and ambulation as permitted by the care plan.
- Offer emotional support and comfort through conversation and interaction.
- Maintain accurate records of observations and interactions; document safety checks every 15 minutes for patients requiring 1:1 supervision.
- Follow all hospital policies and procedures, particularly those related to patient safety and confidentiality.
- Collaborate with the healthcare team to ensure a safe and supportive environment for the patient.
- Perform clerical/reception duties at the nursing station and assist with cleaning rooms upon patient discharge.
Requirements
- High school diploma or equivalent required.
- Previous experience in a healthcare setting or in a role involving direct patient care is preferred.
- Strong observation and attention to detail.
- Excellent communication skills.
- Ability to remain calm and patient in stressful situations; de-escalation skills helpful.
- Empathy and a compassionate approach to patient care.
- CNA Certification preferred but not required.
- BLS Certification required.
- Complete initial on-the-job training and annual competency training, including training for patients at risk for harm or high fall risk.
