ED Clinical Social Worker, 12 Hr Saturdays 9a-9:30p - Per Diem Days

About the Role

Interviews and evaluates patients and family members to identify social, emotional, and economic factors that may interfere with obtaining maximum benefits from medical care. Develops and implements a social work treatment plan for patients and families, selects and utilizes community resources, and makes referrals to coordinate with other professional disciplines or agencies. Provides psycho-social therapy using a variety of treatment methods and participates in multidisciplinary health team conferences to interpret the psycho-social aspects of the patient’s illness.

Provides discharge planning by collaboratively working with the RN Case Manager to determine level of care needs beyond acute/emergency care, offering decision support to patients, families, and physicians, managing expectations, and ensuring a smooth transition to the next level of care. Additional duties include clinical supervision for unlicensed social workers, documentation review, precepting for MSW students, and staff education on social work services.

Responsibilities

• Collaborate with RN Case Managers on discharge planning, including determining post-acute care needs and ensuring smooth transitions.
• Provide clinical supervision for unlicensed social workers.
• Review documentation and precept MSW students.
• Educate and train staff regarding social work services.

Requirements

• Master’s degree in social work (required).
• Evidence of continuing education.
• Knowledge of regulatory requirements related to child/elder abuse reporting, substance use/abuse, child protective custody, adoptions, and other legal mandates.
• Proficiency with mainframe and personal computers.

Skills

• Demonstrated skill in patient assessment, planning, intervention, and evaluation.
• Strong verbal, non-verbal, written, and oral communication skills.
• Familiarity with regulatory agency requirements, policies, and protocols.
• Sound decision-making and prioritization skills.
• Ability to resolve biophysical, psychological, educational, and environmental needs of patients and significant others.

Preferred Experience

• Experience in healthcare.

Pay

$46.00 per hour.

Schedule

Weekend shift.
