About the role
Cadence Care Network is seeking an Early Intervention Service Coordinator to provide support and consultation services to referred families of infants and toddlers with disabilities or developmental delays in Franklin County, OH.
Responsibilities
- Take primary responsibility for the development and implementation of the Individual Family Service Plan (IFSP).
- Coordinate all necessary services, treatments, referrals, and assessments.
- Work in collaboration with community providers and provide referrals for additional services the family may need.
- Attend weekly team meetings with community providers to ensure continuity of care.
- Attend trainings with community partners and stakeholders as it relates to Early Intervention.
Requirements
- Bachelor's degree in a human service-related field.
- Experience working with children ages 0-3.
- Valid Driver License, Auto Insurance, and provision of a vehicle for work purposes.
