Responsibilities and Duties for Early Intervention Case Manager
- Consumer assessment
- Service linkage
- Cordination and development implementation of a case management ISP
- Assist and provide follow up instruction
- Parent education
- Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits
- Manage consumer records according to all Licensure and Medicaid standards
Qualifications
- Bachelor's Degree from an accredited college or university with major course experience in Psychology, Social Work, or related field
- One year of clinical experience in a Human Services field
- Early Intervention Certification preferred
- Valid driver’s license with acceptable DMV driving record at the time of employment and continuously, thereafter
