We are looking for a skilled Physical Therapist (PT) to join our team on a full-time, part-time or PRN basis, it’s a chance to make a meaningful impact on lives each day, with the support of a team who shares your passion for high-quality care. Your Role And Responsibilities As a Physical Therapist, you will: Assess and Evaluate: Conduct thorough evaluations of patients' muscle strength, mobility, gait, range of motion, and potential for rehabilitation.Develop Care Plans: Create personalized treatment plans in coordination with physicians and the care team, tailored to the needs of each patient.Deliver Therapy: Provide direct physical therapy treatments and guide patients in the use of prosthetic devices and therapeutic appliances.Instruct and Educate: Teach patients and their families/caregivers about exercises, treatment goals, and therapeutic techniques.Support Home Care: Offer specialized care and consultation for patients in their home environments, helping them regain function and independence.Document and Report: Maintain accurate clinical records, report patient progress, and participate in discharge planning.Supervise and Coordinate: Supervise physical therapy assistants and collaborate with other team members to ensure the highest quality of care. What We’re Looking For We seek a Physical Therapist who is passionate about improving patients' quality of life and has the following qualifications: A degree in physical therapy from an accredited program.A valid license to practice as a Physical Therapist in the state.At least two years of clinical experience as a physical therapist, with home health or hospice experience preferred.Strong communication, organizational, and documentation skills.Current CPR certification.A valid driver’s license and access to a reliable, insured vehicle.
