Discharge Planning Assistant
Nature and Scope
This position will be required to attend unit rounds on a daily basis to receive direction and communicate progress of all discharge planning activities. Initiates referrals and follow up calls for home care agencies, infusion, wound care providers and Durable Medical Equipment, skilled nursing, and hospice companies as requested by the care team, and arranges transportation. After receiving training may assist with issuing the Medicare Important Message to appropriate patients. The Discharge Planning Assistant must maintain effective communication between the Hospital Care Management Team and community partners. This position is also responsible to document all discharge planning activities appropriately in the medical record.

This position is responsible for sending required information to insurance payers. Managing of multiple fax lines. Monitoring of various reports, and insurance portals for authorizations of inpatient stays, and escalating to Case Manager as needed. Verify patients have admission orders.
Minimum Qualifications
- Name: Education - Must have working-level knowledge of the English language, including reading, writing and speaking English. Understanding of medical terminology preferred.
- Name: Experience - Two years clerical experience in the medical field preferred.

- Name: Computer / Typing - Must possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc. Computer skills necessary to access epic and other supporting computer applications.
