Discharge Planner - Mitchell

The Discharge Planner serves as a liaison between the inpatient setting and the assigned clinician at the Outpatient site, working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Mitchell clinic.
Responsibilities
- Review presenting clinical information, identify resources, and help coordinate successful discharge into the community and follow up with the clinical team post discharge.
- Perform activities in both inpatient and outpatient settings (e.g., inpatient visits, coordination meetings, service planning).
- Connect individuals receiving Level I inpatient services with ongoing Terros Outpatient treatment upon discharge.
- Work with clients while in the hospital, hospital unit staff, outpatient team, member family, PCPs, and health plans as clinically indicated to ensure an effective transition.
- Participate in individualized service planning and execution of written discharge plans.
- Offer coordination to an array of services that actively address recovery goals.
- Reduce the risk of readmission to hospital, reduce the utilization of Crisis services, and improve coordination with the PCP and Terros psychiatric provider.
Schedule
Full-Time: 40 hours/week, Monday through Friday.
