About the Role

Responsible for coordinating various activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker. Assist with development and implementation of discharge plans, case management referrals, and authorizations.

Responsibilities

• Maintain and coordinate Medicaid state submission forms for presentation to on-site reviewer and file them per facility procedure.
• Assist with Discharge Planning by interviewing patients/families and gathering data to develop a safe, patient-focused Discharge Plan.

Requirements

• Knowledge of Discharge Planning, Utilization Management, and Case Management terminology and functions in both managed care and non-managed care environments.
• Basic to intermediate computer skills.
• High School Diploma required.

Qualifications

• Current BCLS certificate required upon hire (maintain current preferred).
• Associates degree or LVN license preferred.
• Experience in a hospital setting preferred.
