Responsibilities

  • Maintain and coordinate Medicaid state submission forms for presentation to on-site reviewer and file them accordingly or as per facility’s procedure.
  • Aid in case management referrals, discharge planning as assigned, by interviewing patients/families, following up on required assignments, and organizing the development of a safe patient-focused discharge plan.
  • Provide clear verbal and written communication of all discharge planning information to stakeholders.

Qualifications

  • Knowledge in discharge planning/utilization management/case management terminology and functions, in both managed care and non-managed care environments.
  • Active state LVN license required upon hire and maintain current.
  • Current BCLS (AHA) or (ARC) certificate required upon hire and maintain current.
  • Experience in basic to intermediate computer skills/knowledge.

Pay

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $25.00 to $36.21. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
