Responsibilities
- Initiates discharge planning for patients, working closely with the treatment team to schedule appropriate after care appointments and communicate with referral sources.
- Carefully coordinates, conducts, and implements discharge planning for patients at the levels of care being provided upon discharge.
- Works as a part of the treatment team and maintains effective communication with teammates and community resources, ensuring accountability for services provided.
- Provides appropriate and timely discharge planning from the facility, working closely with Utilization Review staff to maximize reimbursement and maintain appropriate census at each level of care.
- Creates and maintains relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders, as well as, identifies the need to return clients to referral source at time of discharge.
- Accurately assesses patient's financial resources, including insurance benefits, and coordinates transitions to providers or other referral sources as deemed appropriate.
- Documents and completes discharge plan in EHR, completing final documentation within two working days of discharge.
- Functions as a member of a multidisciplinary treatment management team and the clinical department.
- Ensures a discharge plan, including stable housing, is set in place before discharge.
- Provides exceptional customer service to all internal/external customers with rapid response to inquiries and concerns & support facility internal and external customer service standards.
- Attends team meetings and completes all training courses timely as required.
- Performs other duties as assigned.
Requirements
- Bachelor’s Degree from an accredited college or university, in related field; applicable experience may be substituted for a degree.
- 3-5 years clinical case management/discharge planning experience.
