Discharge Planner
Knapp Medical Center, a 227-bed not-for-profit hospital in Weslaco, Texas, specializes in emergency care, orthopedics, obstetrics and gynecology, wound care, surgery, cardiology, gastroenterology, critical care, and interventional radiology. As the only acute-care hospital in the Mid-Valley area, Knapp is also Weslaco’s only Level III Trauma Center, Advanced Primary Stroke Center, and Level II Newborn Nursery. The hospital has served Mid-Valley residents for 60 years and employs over 450 people.
About the role
We are seeking a Discharge Planner (also referred to as a Hospital Discharge Planner, Patient Care Coordinator, or Patient Services Coordinator) in our Social Work Services Department. In this role, you will support patients, families, and healthcare staff by addressing psychosocial, emotional, and practical needs during hospitalization and after discharge.
Responsibilities
- Coordinate activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker.
- Assist with development and implementation of discharge plans.
- Handle case management referrals and authorizations.
- Maintain and coordinate Medicaid state submission forms for presentation to on-site reviewers and file them according to facility procedures.
- Assist with Discharge Planning by interviewing patients/families and gathering data to develop a safe, patient-focused Discharge Plan.
Requirements
- Knowledge of Discharge Planning, Utilization Management, and Case Management terminology and functions in both managed care and non-managed care environments.
- Basic to intermediate computer skills/knowledge.
- High School Diploma required.
Qualifications
- Current BCLS certificate required upon hire (preferred).
- Associates degree or LVN license preferred.
- Experience in a hospital setting preferred.
