About the role

The Expert Discharge Plan Manager at UPMC Mercy is responsible for coordinating post-discharge needs and ensuring timely utilization of hospital resources. They also serve as the central point of contact for patients, engage with the care team, and facilitate safe and efficient discharges.

Responsibilities

  • Identify clinical, psychosocial, historical, financial, cultural, and spiritual needs to guide the planning process with the patient.
  • Complete detailed assessments on every patient to establish understanding of medical and social factors, determine patient’s capacity for self-care, identify support systems, outline barriers to discharge, and determine likelihood of requiring post-hospital services.
  • Facilitate teams to develop and execute safe and efficient discharges, maintain knowledge about area resources, and ensure appropriate arrangements for post-hospital care before discharge.
  • Engage in clear communication with the patient/member/caregivers and the interdisciplinary care team to develop discharge plans, serve as a liaison between the patient and the care team, and actively collaborate with the care team to coordinate an individualized plan of care.
  • Serve as a contact between hospitals and post-hospital care facilities, recognize and demonstrate shared accountability in the development of a discharge plan, and advocate on behalf of patient/family/caregivers for services access and protection of health, well-being, safety, and rights.
  • Manage cost of care with the benefits of patient safety, clinical quality, risk, and patient satisfaction to provide recommendations and decisions that ensure optimal outcomes.
  • Embrace and incorporate innovation and technology to improve collaboration and patient outcomes, document care in patient medical chart, and lead process improvement initiatives.

Qualifications

  • At least four years of experience in discharge planning/care coordination, including coordination of clinical care needs in various settings such as inpatient, outpatient, post-discharge facilities, home or assisted/skilled living facilities, rehab, hospice, and conducting insurance authorizations.
  • RN Qualifications: BSN required.
  • Social Worker Qualifications: Master’s degree in social work or another health or human services field required.
  • Licensure, Certifications, and Clearances: CCM or ACM or other nursing or social work certification required; RN License required; LSW/LCSW or other related healthcare professional license required.
