Discharge Facilitator | Case Management| Day | Full Time

Facilitate the transition of patients and families between care environments by providing a comprehensive, continuous plan of care—including arranging transportation and preparing information to move patients to the most appropriate level of care.

Schedule

Monday–Friday, 8:00 AM to 4:30 PM

Requirements

• BSW, Bachelor’s degree, or LPN required
• Screening and assessment skills
• Expertise in care coordination and/or utilization management in a health care setting
• Five or more years of related experience may substitute for the credentials

Licensure / Certification / Registration

Active and current appropriate licensure through the Department of Professional Regulation, State of Florida as indicated.

Motor Vehicle Operator Designation

Employees in this position will not operate vehicles for an assigned business purpose.
