About the Role

This position assists the Case Manager and Social Worker with non-professional tasks, including arranging post-discharge services, coordinating Medicare notification letters, and performing clerical duties specific to case management. The role involves effective communication with patients, families, and the care team, as well as compliance with regulatory standards.

Responsibilities

  • Arrange post-discharge services at the direction of the case manager or social worker, including documentation of activities.
  • Assist with clerical duties such as faxing, phone calls, and copying medical records.
  • Coordinate Medicare notification letters to comply with regulatory standards.
  • Maintain effective relationships with case management staff, physicians, nurses, ancillary staff, payors, post-acute vendors, patients, and their families.
  • Attend hospital orientation and workshops focused on the Tenet Case Management Program, discharge planning, and other case management-specific duties.

The following metrics may be used to evaluate performance:

  • Avoidable days related to delays in arranging post-discharge services.
  • Disputes related to delays in discharges.
  • IM letter oversight compliance per policy.

Requirements

  • One to three years’ experience as a discharge planner.
  • Organized with excellent verbal and written communication skills and computer literacy.
  • Knowledge of medical terminology (preferred).
  • Some college education (preferred).

Skills

Proficiency in the following software is required:

  • Allscripts
  • Patient Medical Record/HPF
  • Cerner

Pay

$25.00 - $25.71 hourly. Individual wages are determined based on qualifications and experience.

Physical Requirements

While performing the duties of this job, the employee is regularly required to:

  • Sit, talk, and hear.
  • Use fine motor skills (typing/data entry) and reach with hands and arms.
  • Stand, walk, and occasionally stoop, kneel, or crawl.
  • Regularly lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds.

The individual works in a clinical and office environment.
