What to expect:
- Collaborates with the patients' interdisciplinary treatment team
- Performs aftercare and discharge planning services tailored to the mentally ill patient population
- Maintains a comprehensive knowledge base of agencies, organizations, and resources within the referral community for discharge planning and placement
- Communicates with patients based on an understanding of their developmental needs
- Develops and assists in the implementation of discharge and aftercare plans
- Works with patients and families to facilitate the community transition and ensure appropriate match to patients' needs

- Pay for this position is determined on a number of factors, including but not limited to, years and level of related experience
What we need from you:
- Bachelor's degree in a related mental health care field, such as Social Work, Nursing, Psychology, or a Rehabilitation Services specialty
- For internal candidates, high school education plus an additional 5 years of relevant experience (2 years of work must be within Sheppard Pratt) may be considered a substitute for a bachelor’s degree
- Knowledge of psychiatric terminology, discharge and aftercare planning methods, and community referral resources - acquired through six to twelve months of relevant field or work experience in a psychiatric setting
