Disability Support Coordinator

This position includes a $2,500.00 gross signing bonus at hire and a $2,500.00 gross bonus at 12 months or upon completion of the probationary period.

About the role

The Disability Support Coordinator (Developmental Disability Specialist II) provides direct service coordination for individuals with developmental disabilities receiving home and community-based services under the DD Medicaid Waiver. The role ensures services are available and accessible for individuals to live and thrive in the Arlington community.

The Support Coordinator develops person-centered Individual Support Plans (ISPs) that direct outcomes across major life areas such as meaningful day, health and safety, employment, civic engagement, and community integration. The incumbent will join a collaborative team of Support Coordinators, program managers, and contract managers that facilitate person-centered planning for individuals supported by the program.

Responsibilities

• Provide direct service coordination for individuals with developmental disabilities under the DD Medicaid Waiver.
• Ensure services are accessible and support individuals in living and thriving within the Arlington community.
• Develop person-centered Individual Support Plans (ISPs) that address outcomes in areas such as meaningful day, health and safety, employment, civic engagement, and community integration.
• Collaborate with a team of Support Coordinators, program managers, and contract managers to facilitate person-centered planning.
