About the role
Supporting the mission of Habitat Health, we are seeking individuals who can help us achieve our vision of promoting vitality for older adults. This role involves working closely with clients, their families, and healthcare providers to ensure they receive comprehensive care services.Responsibilities
- Assess client needs and develop personalized care plans
- Coordinate with healthcare providers to provide necessary medical and social services
- Facilitate communication between clients, families, and healthcare teams
- Ensure compliance with regulatory standards and policies
- Provide emotional support and advocacy for clients
Requirements
- Bachelor’s degree in healthcare, social work, gerontology, or related field
- Minimum of 2 years of experience in healthcare administration or related field
- Strong interpersonal and communication skills
- Ability to manage multiple tasks and prioritize effectively
- Knowledge of PACE program principles and regulations
Qualifications
- Valid driver’s license and reliable transportation
- Proficiency in Microsoft Office Suite
- Excellent organizational and time management skills
