Director of Social Work

Executive Search Network/Platinum Staffing is seeking a highly skilled and experienced Director of Social Work to join a skilled nursing, long-term care facility located in Suffolk County, New York.

About the role

As a key member of the team, you will provide exceptional social services to residents and lead and manage a team of social workers.

Responsibilities

• Assist new residents and their families in the initial adjustment phase and ongoing transition to institutional living.
• Provide information to residents and/or family members on residents' rights and responsibilities.
• Interview residents/families to obtain social history.
• Participate in discharge planning, development and implementation of social care plans, and resident assessments.
• Interpret social, psychological, and emotional needs of the resident care team members.
• Involve the resident/facility in planning objectives and goals for the resident.
• Refer residents/families to appropriate social services agencies as needed.
• Arrange transportation to other facilities when necessary.

Requirements

• Bachelor's degree in Social Work or related field.
• Minimum 2 years of experience as a Social Worker in a Nursing Home.
• Experience as a Social Worker in a Nursing Home.
• Strong leadership and management skills.
• Excellent written and verbal communication skills.
