Director of Social Services | LSW Required - $10,000 Sign On Bonus!

About the role

The Social Worker assists in providing social services to residents and their families to help them deal with the impact of illness and extended care placement. This position participates in resident care management, ensuring needs are met through Social Service programming and appropriate referrals to outside agencies and community resources.

Responsibilities

• Identify the medically-related social service needs of residents and assist with ensuring those needs are met by the appropriate discipline.
• Maintain contact with family (with resident’s permission) to report on changes in behavior, psychosocial functioning, discharge planning, and encourage participation in care planning.
• Assist residents/responsible parties with financial and legal matters such as drafting petitions for guardianship/conservatorship and pre-paid funeral arrangements.
• Coordinate discharge planning services, including placing residents on waiting lists for community congregate living, arranging home care services, and assisting with transfers to other facilities.
• Through assessment and care planning, identify and support residents’ individual needs, preferences, routines, concerns, and choices.

Requirements

• Bachelor’s Degree in Social Work or a related human services field (e.g., Sociology, Special Education, Rehabilitation Counseling, Psychology).
• Minimum of a Limited License in Social Work (LBSW preferred).
• 1+ years of supervised social work experience in a health care setting working directly with individuals.
• Excellent customer service skills with a commitment to delivering Excellent, Passionate, Innovative care.
• Team player dedicated to supporting colleagues and completing tasks effectively.

Pay

Starting salary of $73,000 per year, based on experience, plus a $10,000 sign-on bonus (paid out over one year). Annual salary increases are provided.
