Responsibilities
- Comply with all applicable rules, policies, standards, and guidelines related to employment with Westminster Services and its communities.
- Prepare comprehensive psychosocial assessments and care plans for all residents.
- During the admission process, tour the facility and available accommodations with potential residents and family members.
- Review and obtain signatures for required documentation.
- Serve as the main contact for resident and family outreach, advocating for residents.
- Provide access to community resources and present residents/family members with healthcare provider availability, including home health care, hospice, and equipment providers.
- Manage discharge planning, ensuring appropriate discharges to less restrictive settings (e.g., outside health services).
- Act as the internal contact for resolving resident and family member grievances.
- As Grievance Coordinator, maintain a Grievance Log, accept/write grievances on behalf of parties, conduct investigations, and follow up with resolutions.
- Conduct abuse prevention in-services for staff members.
- Complete QA and audits.
- Monitor psychotropic medications, mood, and behavioral symptoms.
- Obtain consent from residents or healthcare surrogates for psychotropic medication use and maintain records in the psychoactive medications binder.
Requirements
- Bachelor’s Degree in Social Work required; Master's Degree in Social Work preferred.
- LCSW (Licensed Clinical Social Worker) certification.
- 3 years of experience in a skilled nursing facility (long-term care) or medically related area.
