About the Role
Join our team as a Social Worker and work alongside fellow caregivers to provide high-quality patient care in a fun and supportive work environment.
Responsibilities
- Assess nature of patients' situations by interviewing and reviewing personal history
- Develop and execute individual treatment plans/care plans
- Determine appropriate milestones to gauge patient progress
- Communicate patient updates to all relevant parties
- Facilitate referrals to other healthcare professionals and programs as needed
- Maintain accurate documentation
Requirements
- Previous experience in social work, with a focus on short-term and long-term care patients
- Compassionate and caring demeanor
- Ability to build rapport with staff, patients, and their loved ones
- Strong leadership qualities
- Excellent written and verbal communication skills
