Director of Resident Care (LPN Required)

About the Role

Leads the day-to-day operations of high-quality, person-centered care within the community, completing residential placement and ongoing nursing processes. Collaborates with physicians and multidisciplinary team members to provide social, emotional, physical, and psychological support to residents, friends, and families. Assists with hiring, training, and supervising assigned care team members. Communicates daily with the Executive Director and assumes authority to carry out the licensee or Executive Director’s responsibilities in their absence.

Responsibilities

• Sets the tone and standards for both person and relationship-centered care reflecting the core values of Vibrant Life.
• Leads overall day-to-day medical operations.
• Steps in as a Med Tech/Resident Assistant when needed to adhere to licensing requirements and Vibrant Life Standards.
• Identifies resident care requirements by establishing personal rapport with potential and actual residents and others in a position to understand care requirements.
• Establishes a compassionate and vibrant environment, providing emotional, psychological, and spiritual support to residents, their friends, and families.
• Ensures proficiency in state regulations and other licensing requirements.
• Directs and supervises scheduling of direct care employees within established guidelines.
• Leads training for care staff and partners with other management team members in ongoing staff recruitment, retention, and development.
• Promotes person-centered care goals; teaches residents (as able), staff, family, and friends to understand conditions, medications, and self-care skills; answers questions.
• Assures quality of care by adhering to established dementia care standards; measures health outcomes against person-centered care goals and standards; makes or recommends necessary adjustments.
• Resolves residents' problems and needs by utilizing multidisciplinary team strategies.
• Maintains a safe and clean working environment by complying with procedures, rules, and regulations.
• Protects residents and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
• Documents individual care service plans by charting in resident records and department records.
• Maintains continuity among staff teams by documenting and communicating actions, irregularities, and continuing needs.
• Maintains resident and family confidence by keeping information confidential.
• Maintains nursing supplies inventory by checking stock, anticipating needed supplies, placing and expediting orders, and verifying receipt.
• Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
• Maintains cooperative relationships among healthcare teams (both internally and externally) by communicating information, responding to requests, building rapport, and participating in team problem-solving methods.
• Works with the Executive Director to assure efficient, cost-effective operation of the community.
• Ensures operation of equipment by completing preventive maintenance requirements, troubleshooting malfunctions, calling for repairs, and evaluating new equipment and techniques.
• Consistently meets or exceeds regulatory service and care requirements.
• Provides consultation and phone triage after business hours to staff for nursing needs beyond their scope of knowledge.
• Attends and participates in meetings and training.
• Completes and maintains all state-required forms for the nursing department.
• Carries out referrals for DME services, primary physician, Hospice, PT, and PACE orders.
• Makes medication changes as needed and communicates with pharmacies regarding medication orders.
• Manages narcotic control through tracking, destruction, and refills.
• Monitors non-controlled medications such as discontinuation and charting into ECP.
• Orders labs and reports results.
• Assists with scheduling and hiring care staff to stay in state compliance.
• Performs other duties as assigned by the Executive Director.

Requirements

• High School Diploma or equivalent required.
• A minimum of 3 to 5 years of previous experience as an LPN.
• Experience in Senior Living Management with special knowledge/expertise in Dementia Care.
• Must have a desire to work with Dementia/Alzheimer’s.
• Must be able to read, write, and speak English fluently.
• Ability to work flexible hours as needed.

Physical Requirements

Safe body mechanics is an expected part of this position, including frequent bending, lifting, walking, sitting, and using hands in repetitive motion.
