Director of Rehabilitation (DOR) | Skilled Nursing Facility
Job Summary
As the Director of Rehabilitation (Therapy Director/Rehab Manager) at Edgerton Care Center, you will provide leadership and oversight for our rehabilitation services program, ensuring the delivery of high-quality, resident-centered care that helps individuals achieve their highest level of independence and quality of life. In this role, you will lead and support a team of therapy professionals, coordinate interdisciplinary care, and ensure compliance with all applicable federal, state, and local regulations. The ideal candidate is a collaborative healthcare leader who is passionate about clinical excellence, team development, and creating positive outcomes for residents.
Compensation and Benefits
Compensation: $40.00 to $47.00 per hour dependent on experience

Key Responsibilities
- Meet all the Duties and Responsibilities of Discipline Specific Job Description
- Attendance of Designated Facility Meetings
- Attendance of Necessary Resident and Family Meetings
- Effective Management of Rehabilitation Department
- Entering of New Patient Information into Net Health to facilitate Therapy Evaluations
- Entering of Correct Medicare Information (GG/PDPM/Projections)
- Scheduling of Therapists
- Validation of Payroll
- Partnering with Administrator to Meet Department Financial and Clinical Goals
- Ensuring Clinical Documentation is Timely and in compliance
- Review of Care Plans with Nursing Department
- Ensuring Restorative Plans Are Up To Date
- Providing Direct Patient Care at Appropriate Levels for Size of Department
- Maintaining of Staff Credentials and Certifications
- Completing Other Duties as Assigned
Qualifications/Requirements
- Demonstrate and Understanding of Leadership, Long Term Care and Program Development
- A valid, unrestricted License in his/her therapy discipline in the state of Wisconsin.
- Meet educational requirements of his/her licensure requirements.
- Valid driver’s license
- Must have at least three (3) years of experience in a supervisory role in long-term care
- Knowledgeable of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long term care
