Responsibilities

  • Serve as a role model and resource for nursing staff and interdisciplinary team members
  • Support staff in problem-solving and determining appropriate clinical interventions
  • Collaborate with the Administrator on hiring, orientation, retention, and performance management
  • Adhere to budget requirements for labor, pharmacy, DME, and medical supplies
  • Coordinate patient care with RN Case Managers and attending physicians
  • Review and manage patients receiving higher levels of care (respite, inpatient, continuous care)
  • Oversee clinical and management responsibilities for assigned teams
  • Serve as liaison between Administrator, clinical staff, patients, families, and community partners

Qualifications

  • Graduate of an accredited school of nursing with a bachelor’s degree or higher in nursing or a related health field
  • Current, unencumbered Registered Nurse (RN) license in the state of employment
  • Active CPR certification required; certification in Hospice and Palliative Nursing preferred
  • At least 4 years of full-time experience within the last 5 years in hospice, palliative care, supportive care, home health, or a primary care setting
  • Minimum of 2 years of supervisory or managerial experience in a hospice or home health setting within the past 5 years
  • Experience with Electronic Medical Records (EMR) preferred; familiarity with WellSky is highly desirable
  • Valid driver’s license with a clean driving record
  • Reliable transportation and willingness to travel as needed
  • Able to lead bereavement support groups and make sound clinical decisions
  • Flexibility to work outside regular hours based on patient needs

Pay

Anticipated pay range: $140,000 USD - $155,000 USD
