We’re hiring a Director of Patient Care Services – a compassionate and experienced clinical leader who oversees hospice care delivery, supports interdisciplinary teams, and ensures regulatory compliance. This role blends leadership, clinical expertise, and empathy to provide dignified, meaningful care to patients and families.
Responsibilities
- Serve as a role model and resource for nursing staff and interdisciplinary team members
- Support staff in problem-solving and determining appropriate clinical interventions
- Collaborate with the Administrator on hiring, orientation, retention, and performance management
- Adhere to budget requirements for labor, pharmacy, DME, and medical supplies
- Coordinate patient care with RN Case Managers and attending physicians
- Review and manage patients receiving higher levels of care (respite, inpatient, continuous care)
- Oversee clinical and management responsibilities for assigned teams
- Serve as liaison between Administrator, clinical staff, patients, families, and community partners
Qualifications
- Graduate of an accredited school of nursing with a bachelor’s degree or higher in nursing or a related health field
- Current, unencumbered Registered Nurse (RN) license in the state of employment
- Active CPR certification required; certification in Hospice and Palliative Nursing preferred
- Must meet one of the following:
- Bachelor’s degree or higher in nursing or a related health field and at least 2 years of full-time supervisory or managerial experience in a hospice or home health agency within the last 5 years
- At least 4 years of full-time supervisory or managerial experience in a hospice or home health agency within the last 5 years
- Experience with Electronic Medical Records (EMR) preferred; familiarity with WellSky is highly desirable
- Valid driver’s license with a clean driving record
- Reliable transportation and willingness to travel as needed
- Ability to lead bereavement support groups and make sound clinical decisions
- Flexibility to work outside regular hours based on patient needs

Pay and Schedule
Schedule: Monday to Friday, 8:00am - 5:00pm, rotating administrator on-call
Anticipated pay range $115,000—$125,000 USD
