Director Of Nursing - LPN

The Grove at Trelago is operated by Starling, based in Jacksonville, FL. Starling is an owner-operator of multiple properties within the Southeast Region.

About the role

We’re looking for a dynamic and personable Director of Nursing to manage all care aspects of this 24-hour, 7-day-a-week assisted living community, ensuring resident and family satisfaction as well as regulatory compliance. The Director of Nursing will work closely with the Executive Director, Starling’s COO, and the Regional team to develop and execute initiatives that consistently deliver the company’s Mission, quality standards, and Core Values.

Responsible for providing overall leadership and management of the health and well-being of the residents within the community.

Responsibilities

• Ensures that the community follows all federal, state, and local laws and regulations as they pertain to clinical care.
• Responsible for collecting, analyzing, and reporting occurrence trends, and if necessary, developing and implementing improvement plans.
• Partners with the Executive Director and other team members to analyze and maintain all resident and team member health safety.
• Partners with the pharmacy consultant to provide optimal pharmaceutical services to residents.
• Provides clinical expertise for licensed nurses.
• Assists with educational presentations as assigned by the administrator.
• Identifies ongoing needs and services of residents through the evaluation and Service Plan process, including documentation for residents with a change in health care status.
• Oversees the completion of monthly updates and reports any pertinent changes to the responsible party.
• Partners with other department coordinators to identify, review, and discuss potential changes in service or medication levels if applicable.
• Ensures the resident’s service plan is updated as indicated by state regulations.
• Participates in community awareness activities and community relations.
• Maintains communications with residents’ families and/or responsible parties regarding changes in care or health concerns.
• Informs the Business Office of fee changes related to care needs, if applicable.
• Ensures adequate clinical supplies are available.
• Participates in a rotating on-call schedule.
• Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws, including interviewing, hiring, training employees, planning, assigning, and directing work, appraising performance, rewarding and disciplining employees, and addressing complaints and resolving problems.
• Other tasks as assigned.

Requirements

• Current state license as a Licensed Professional Nurse or a Professional Registered Nurse per state regulations.
• Must demonstrate competence in assessment skills, injections/medication administration, follow-up, and triage.
• Experience in assisted living, home health, or long-term care industries.
• Two (2) years of experience as a Licensed Nurse.
• One (1) year of nursing management experience.
