Director of Nursing

About the role

The Terraces at Los Altos is a nonprofit Life Plan Community located in a quiet residential neighborhood near downtown Los Altos. We are seeking an experienced and compassionate Director of Nursing (DON) to lead the Skilled Nursing Health Center.

Responsibilities

• Conducting routine rounds throughout the Health Center to monitor resident well-being, assess quality of care, and confirm that staffing patterns align with current resident needs and acuity levels.
• Serving as a primary point of contact for residents, families, and staff, providing guidance, addressing concerns, and fostering a supportive and collaborative environment.
• Ensuring the delivery of comprehensive and appropriate nursing services consistent with resident care plans, regulatory standards, and evidence-based practice.
• Overseeing admissions, transfers, and discharges, coordinating closely with partner hospitals and teams across Assisted Living, Memory Support, and Independent Living for seamless transitions of care.
• Leading, supervising, and developing nursing and department leadership teams, including responsibility for hiring, onboarding, scheduling, coaching, performance evaluations, and addressing staff feedback and concerns.

Requirements

• Active California Registered Nurse (RN) license and graduation from an accredited nursing program.
• CPR certification.
• Bachelor’s degree in nursing required; master’s degree preferred, or a comparable blend of education and relevant professional experience.
• 3–5 years of progressive nursing leadership experience, ideally in a Skilled Nursing Facility or related care environment.
• Strong clinical assessment and critical-thinking abilities, with a keen eye for detail and commitment to best-practice standards.
• Proficiency with electronic health records (EHRs) and comfort navigating computer-based documentation, communication platforms, and reporting tools.

Qualifications

Pay

Annual salary: $135,000 - $175,000 (DOE) plus performance-based incentives.
