About the role
Direct, supervise, and coordinate patient care services for a home healthcare agency. Reports to the Administrator and is part of the Clinical Department.
Responsibilities
- Be available at all times during the agency's operating hours
- Be able to read, write, and comprehend English
- Participate in development of qualifications for, assignment of, and counseling of agency staff
- Assure that the plan of care is executed as written
- Assure re-assessment of the patient's needs is performed by the appropriate health care professional when there is a significant health status change in the patient's condition, at the physician's request, or after hospitalization
- Assures appropriate follow-up and re-evaluation of patient's condition change by an RN as necessary
- Other duties as stipulated in the job description of the Director of Nursing, Clinical Manager, and QAPI Director
- Drive to all branches to supervise all applicable staff and attend meetings
- Assist in start-up process of new branches
Requirements
- RN license, in good standing
- Reliable forms of transportation and communication
- Effective problem solving and communication skills
- Proficient with technology including the ability to learn how to effectively/efficiently use an EMR system
- Knowledge of and passion for home healthcare is preferred
- Management experience is preferred
- Able to sit at a desk for up to 8 hours per day
- Able to lift/move up to 25 pounds
- Able to work with staff
- Able to work in a stressful environment
- Able to perform physical tasks, such as bending, standing, and lifting
- Able to drive 120 miles
Qualifications
- Meet the requirements and be licensed in the State of Pennsylvania
- Be a Registered Nurse and a qualified licensed professional
- Have at least one year of experience in nursing obtained within the last 36 months
- Evidence of maturity and ability to deal effectively with job demands
- Ability to deal and work effectively in a stressful environment
- Demonstrate ability to read, write and carry out directions
- Able to organize and file in alphabetical and chronological order
- Time management, organization and attention to detail skills
- Knowledge of and willingness to learn basic office procedures and use of equipment
- Proof of basic computer skills and knowledge
- Employment qualification requirements remain in compliance throughout the year
- Shall have a criminal history background check (FBI, State & Child Abuse, OIG and Social Security number check prior to being offered employment with this agency)
- Neat in appearance and practice, with good personal hygiene. Maintain dress code policy
- Maintain confidentiality regarding all agency records
- Ability to remain in compliance with all agency policies and procedures
- Driver's license, in good standing
Schedule
Full-time position. Typical working hours are during regular office hours, Monday-Friday, 8:30am-4:30pm. Occasional evening and weekend responsibilities, as needed. On-call availability, as needed.
