About the Role

Manages and monitors the financial performance of the agency, ensuring fiscal responsibility through variance analysis, cost-reduction measures, and optimized reimbursement. Oversees contract service management, including payer credentialing updates. Maintains continuity and coordinates office activities to support departmental standards of care, directing staff per policy and developing procedures to improve operational efficiency. Provides supervision to support positions as assigned and ensures all day-to-day operations run smoothly.

Ensures clinical supervision is available during all operating hours, with an RN accessible 24/7 for patient concerns. Performs clinical supervisor duties, including intake, scheduling, quality review, utilization management, and medical records as needed. Ensures the agency employs qualified personnel meeting state and federal requirements, collaborating with talent acquisition and HR on personnel matters.

Accountable for exceeding national benchmarks in key performance indicators impacting reportable outcomes and financial health. Provides oversight of clinical, operational, and financial functions in compliance with regulatory entities (e.g., state agencies, CMS, CLIA) and accrediting organizations. Collaborates with hospital leaders and care management teams to improve patient outcomes and transitions of care.

Implements a quality improvement program covering patient safety initiatives (e.g., infection control), risk management, performance improvement, patient experience, and trend monitoring. Ensures the QI committee meets quarterly to review data, implement improvement plans, and evaluate results.

Responsibilities

Requirements

  • Bachelor’s degree in Nursing or other healthcare-related field [Required].
  • 5+ years of progressive experience leading home-based care operations or related healthcare operations [Required].
  • An equivalent combination of education and relevant work experience may substitute for the degree requirement (e.g., 4+ years of equivalent experience).
  • Registered Nurse (RN) [Required] OR Physical Therapist (PT) [Required] license.
  • Basic Life Support (BLS) CPR certification [Preferred].

Pay

$96,266.14 - $179,045.63 per year.
