Brief Description

Join a mission-driven hospice organization recognized for providing compassionate end-of-life care throughout Orange and Sullivan Counties! Hospice of Orange & Sullivan is seeking an experienced and compassionate Licensed Clinical Social Worker (LCSW) to lead our Social Work and Bereavement Care teams.

Key Responsibilities

• Provide leadership and supervision to Social Work and Bereavement Care teams
• Co-facilitate interdisciplinary group (IDG) meetings and support care coordination
• Ensure compliance with hospice regulations, accreditation standards, and organizational policies
• Mentor, coach, and develop team members through education, performance management, and professional development
• Oversee bereavement program development, evaluation, and enhancement
• Participate in quality improvement initiatives and organizational leadership activities
• Collaborate with community partners and serve as Chair of the Veteran-to-Veteran Program Committee
• Co-facilitate the training and onboarding of new volunteers

Requirements

• New York State Licensed Clinical Social Worker (LCSW)
• Minimum 3 years of management experience, preferably in hospice, home care, or healthcare operations
• ACHP-SW certification (or ability to obtain and maintain)
• Strong leadership, communication, organizational, and relationship-building skills
• Valid driver's license with acceptable driving record
