Director of Case Management

A health system in Brooklyn is seeking an experienced Director of Case Management to lead its hospital-wide Case Management function. This is a high-impact leadership opportunity reporting directly to the Chief Nurse Executive, with the ability to strengthen care coordination, discharge processes, and patient outcomes across the organization. The Director of Case Management will be responsible for:Leading Case Management, Utilization Review, Discharge Planning, and Social Work functionsDriving improvements in length of stay, avoidable days, readmissions, and discharge efficiencyLeading interdisciplinary rounds and addressing barriers to safe and timely dischargeOverseeing utilization review, medical necessity, denial prevention, and appealsBuilding partnerships with post-acute, home care, and community-based providers Requirements:Current NYS Registered Nurse (RN) license and Bachelor of Science in Nursing (BSN); Master’s degree preferred5+ years of acute care case management experience, including 3+ years of case management leadershipACM or CCM certification, or eligibility to obtain certificationStrong acute care hospital experience with knowledge of CMS requirements, InterQual and/or MCG, utilization review, discharge planning, and case management metrics Compensation: $175,000 to $200,000
