About the Role

The Supported Living team is looking to expand their team and provide reliable services to our community. We are looking for individuals that encompass Adjoin values of People, Integrity, Innovation, Compassion, and Fun. Ideal candidates are passionate about coaching and supporting individuals in developing the skills needed to enhance their quality of life.

A few key qualifications include a willingness to travel, open availability (including the potential for split shifts), a collaborative mindset, and a strong sense of innovation.

Responsibilities

  • Facilitate the client’s inclusion in positive community experiences.
  • Work comfortably in community settings and remain physically active throughout the day.
  • Transport clients to appointments, community activities, or other locations as needed, ensuring safety and timeliness during travel.
  • Advance client’s decision-making, productivity, and participation in a range of daily activities.
  • Maintain current knowledge of client’s needs.
  • Maintain positive communications and relationships with supervisors, clients, landlords, employers, involved family members, Regional Center, D.O.R., and other individuals in the community.
  • Participate in the evaluation of the client through completion of task analysis and other requested assessment devices.
  • Implement individual training and support programs under the direction of the case responsible person.
  • Document required activity records, reports, incidents, and client evaluations in a timely, complete, and accurate manner as required.
  • Promote independence in daily activities while being mindful of client’s intellectual and physical disabilities; assist with meals, cooking, household maintenance, grooming, and dressing as assigned.
  • Address challenging behaviors proactively and respectfully, using methods outlined in plans and training.
  • Oversee client activities and environment throughout shift to ensure the fulfillment of health and safety needs.
  • Provide direct service as needed and required.
  • Communicate effectively with clients, families, supervisors, and team members.
  • Perform all other job duties as assigned.

Requirements

  • 18 years of age or older.
  • High School Diploma or Equivalent.
  • Successful completion of pre-employment drug screen and background check.
  • Insured, registered vehicle in safe working condition, and proof of current CA driver’s license.
  • Certification in an approved First-Aid and CPR class; or successful completion within 45 days of the date of hire.
  • Positive attitude and strong work ethic.
  • Aptitude for confidentiality, organizational skills, and good interpersonal skills.
  • Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.
  • This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided.
  • Adjoin’s client case management platform requires employees to utilize a mobile smart device (i.e., cell phone/tablet) with reliable internet connectivity and data to complete certain essential job tasks.

A Direct Support Professional works as scheduled by the Regional Manager and/or their designee. Overtime may be required as dictated by operational needs; however, no holiday or overtime may be worked without the prior approval of the Direct Support Professional's supervisor. The schedule may be either part-time or full-time, depending on the needs of the clients and Adjoin.

Pay

$16.90 - $17.10 per hour
