Responsibilities

  • Affords assistance to the patient in achieving the individualized treatment plan goals.
  • Maintains safety, living comfort, and hygiene needs of the patient.
  • Coordinates transportation and escorts the patient to medical/other appointments and outings.
  • Updates staff and family members about scheduled appointments.
  • Enforces the Bill of Rights of the patient and reports violations to management.
  • Facilitates community reintegration activities under the guidance of the clinical and/or residential treatment team.
  • Assists the patient in understanding their needs and limitations.
  • Monitors and documents changes in the patient's condition.
  • Attends case conferences and family educational meetings.
  • Maintains confidentiality and shares information only when necessary.
  • Participates in the Company’s privacy and ethics programs.
  • Maintains accurate documentation and follows regulatory requirements.
  • Maintains a professional appearance, is punctual, cooperative, and adheres to professional standards.
  • Engages in staff development and continuing education.
  • Fosters a safe home environment for the patient.
  • Reports any safety concerns immediately.
  • Performs other duties as assigned.

Qualifications

  • Direct Support Professional: Successfully complete the Direct Support Professional curriculum or demonstrate proficiency through Assessment of Prior Learning, or enroll in the Maine College of Direct Support within six months of hire.
  • Personal Support Specialist: Hold a valid certificate of training as a personal support specialist/personal care assistant, pass the competency-based examination, and complete 50 hours of formal classroom instruction, demonstration, return demonstration, and examination.
  • Meet additional requirements including being a Certified Brain Injury Specialist, having a high school diploma or GED, completing Reportable Events Training, Behavior Regulations Training, adult and child protective record checks, and maintaining a State of Maine Certified Residential Medication Aide status.
  • Demonstrate effective interpersonal, verbal, and written communication skills, strong organizational skills, and attention to detail.
  • Have reliable transportation and be able to travel between residences and office during shifts.
