Direct Support Professional

About the role

Systems Unlimited seeks a Direct Support Professional to join our team. This position provides direct care and support to individuals with disabilities and mental health needs.

Responsibilities

• Provide personal care and assistance to individuals as needed
• Support individuals in activities of daily living
• Assist with medication management
• Facilitate communication between individuals and family members, service providers, and other professionals
• Participate in planning and implementing individualized care plans
• Ensure safety and security of individuals under care
• Document observations and progress in care plans
• Collaborate with interdisciplinary teams to provide comprehensive support

Requirements

• High school diploma or equivalent
• Valid driver’s license and access to reliable transportation (if applicable)
• Experience working with individuals with disabilities and/or mental health conditions preferred
• Strong interpersonal skills and ability to build positive relationships
• Ability to work flexible hours including evenings and weekends
• Passing background check and criminal history review
