Responsibilities

  • Provide direct care to community residents for activities of daily living.
  • Assist residents in and out of bed and into wheelchairs, including use of a gait belt.
  • Assist residents with bathing, dressing, undressing, and peri care.
  • Take and record vital signs according to community schedule and policies.
  • Assist residents to the dining area for meals; may include feeding assistance.
  • Strip and replace bed linens as needed or per schedule.
  • Assist with personal grooming of residents as required.
  • Turn residents as designated on care plans.
  • Clean bedpans and empty catheter bags as needed.
  • Document and report resident information as instructed; complete documentation before leaving shift.
  • Provide shift report on resident status.
  • Maintain med room in a clean and orderly manner.
  • Perform housekeeping and cleaning services in resident and common areas as scheduled.
  • Provide laundry and linen service, including delivery of resident laundry.
  • Perform minor maintenance and servicing on community equipment, including supply inventory.
  • Monitor compliance with state and federal requirements (e.g., bloodborne pathogens, hazardous materials, infection control, fire safety).
  • Monitor compliance with residents’ rights.
  • Available to work rotating weekends and holidays.

Requirements

  • Regular worksite attendance.
  • Complete at least 12 hours of in-service education annually as required by the community, Department of Human Services, and Department of Inspections and Appeals.
  • Complete Relias assignments on time.
  • Successfully complete annual skills check-off with the RN.
  • Demonstrate ability to develop and maintain good working relationships with co-workers and other departments.
  • Basic computer skills.
  • If required to operate a motor vehicle, comply with Company’s Motor Vehicles policy.
  • Ability to read, write, speak, and understand English.
  • Willingness to work beyond normal hours, weekends, and holidays when necessary.
  • Assist in resident evacuation during emergencies.
  • Perform essential job functions with or without reasonable accommodation.
  • Use a transfer/gait belt or seek assistance when lifting or carrying objects over 25 pounds.

Additional physical considerations include twisting, bending, crawling, squatting, kneeling, crouching, climbing, and balancing. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, or hostile individuals. Must have adequate vision and hearing (or use prosthetics) to perform job requirements.
