What You'll Do

• Support individuals with daily living skills such as cooking, hygiene, and routines
• Provide hands-on personal care and mobility assistance as needed
• Assist with and attend medical appointments and community activities
• Implement individualized support and behavioral plans
• Administer medications in accordance with established procedures
• Maintain a clean, safe, and welcoming home environment
• Provide transportation as required
• Complete documentation and progress reports
• Collaborate with team members, families, and care providers
• Serve as a positive role model and advocate for those you support

This is an active, hands-on role supporting individuals both in the home and out in the community. Responsibilities may include physical care, behavioral support (including trained interventions), and maintaining the home environment.

Qualifications

• High School Diploma or equivalent
• Valid Driver's License
• Basic knowledge of Microsoft Word and the internet
• Successful completion and maintenance of all state and agency mandated trainings, including medication administration
• Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies
• Must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions
• Must be fluent in English and have the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations
• Ability to write reports, business correspondence, and procedure manuals
• Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community
• While performing the duties of this job, the employee is occasionally required to stand, walk, and sit; use fingers, handle, or feel; reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl; and talk and hear. The employee is occasionally required to taste or smell
