Job Description
Maintains continuity and coordinates office activities and procedures to support the standards of care established by the department. Directs staff to carry out responsibilities per department policy. Develops and implements policies and procedures to improve operational efficiency. Provides support and supervision to support positions as assigned. Responsible for all day-to-day operations of the agency.
Ensures that a clinical supervisor is available during all operating hours in which the agency is open and providing care to patients and that an RN (Registered Nurse) is available after hours (24/7) to address patient concerns. Performs the functions of the clinical supervisor, intake, scheduling, quality review, utilization management medical records as needed.
Ensures the agency employs qualified personnel that meet all state and federal requirements. Collaborates with the talent acquisition and HR team related to all personnel related matters. Accountable to meet & exceed national benchmarks in key performance indicators that impact reportable outcomes and the financial health of the organization.
Ensures the agency’s clinical, operational, and financial functions comply with regulatory entities (state regulatory agencies, CMS, CLIA (Clinical Laboratory Improvement Amendments), etc.) and accrediting organizations. Interacts regularly with hospital leaders and care management teams to foster relationships, collaborate on patient outcomes and transitions of care.
Ensures a quality improvement program is implemented that includes patient safety initiatives such as infection control; and includes risk management, performance improvement projects, patient experience, and monitors patient trends and outcomes. Ensures the QI committee meets at least quarterly to review data/trends and implement plans for improvement with follow-up to evaluate results.

Education and Experience
- Bachelor's [Required] Field Of Study of Nursing or other healthcare related field
- 5+ years of progressive experience leading home-based care operations or other related healthcare operations [Required]
Licenses and Certifications
- Registered Nurse (RN) [Required]
- Physical Therapist (PT) [Required]
- Basic Life Support - CPR Cert (BLS) [Preferred]

$109,174.68 - $203,054.25
