About the role

Join True Rx Health Strategists and make a significant impact on patients' lives through expert nutrition counseling. As a Clinical Dietitian, you will perform specialized duties related to the nutrition counseling of patients and families for Medical Nutrition Therapy (MNT) and diet education for complex medical conditions such as diabetes, obesity, and cardiovascular disease. The primary focus is to serve clients and members with comprehensive, value-based nutritional care to achieve definitive outcomes that improve a patient's quality of life.

Responsibilities

  • One-on-One Counseling: Provide extensive nutritional and lifestyle education to patients via telehealth to enhance their health and reduce healthcare costs.
  • Sustainable Weight Loss: Work closely with patients to achieve and maintain healthy weight loss.
  • Resource Education: Guide patients to appropriate community resources to support their health journey.
  • Outcome Evaluation: Assess nutritional and lifestyle outcomes, developing impactful reports to track progress.
  • Clinical Recommendations: Formulate and communicate clinical recommendations and visit summaries to healthcare providers and patients.
  • Barrier Identification: Proactively identify and address barriers to effective patient care.
  • Collaborative Development: Partner with departments to leverage technology and health information for better patient outcomes.
  • Client Reporting: Deliver clinical reports and presentations in partnership with the account manager, educating clients on their benefit plans.
  • Program Implementation: Educate clients on clinical programs and solutions to increase adoption and implementation.
  • Team Leadership: Plan, organize, and direct responsibilities as delegated by the Clinical Innovation Pharmacist.

Requirements

  • Bachelor’s degree required; Registered Dietitian Certification (CDR); State Certification or Licensure.
  • Strong work experience counseling individuals in an outpatient setting with complex disease states to prevent complications.
  • Familiarity with current treatment standards and options for common metabolic conditions, including diabetes and obesity.
  • Excellent verbal and written communication skills.
  • Proficiency in MS Word, Excel, and Outlook, with experience creating and updating Excel spreadsheets.
  • Ability to work independently with minimal supervision.
  • Skilled in identifying patient diet/lifestyle barriers and developing strategies to overcome them.
  • Significant experience in patient education and engagement using motivational interviewing techniques.
  • Willingness to travel as required.
