Responsibilities
- Train patients (and/or care partners) in the practice of self-care for Peritoneal Dialysis or Hemodialysis, adhering to federal and state regulations.
- Conduct home visits to assess the patient's home environment, initially and minimally annually thereafter.
- Routinely evaluate patient performance and assess the home environment as needed to enhance care delivery.
- Communicate results of patient assessments, reassessments, and ongoing monitoring to physicians, team members, and other relevant parties.
- Document all nursing services in the Electronic Medical Record, ensuring accurate reflection of patient status and interventions.
- Participate in infection control monitoring, implementation, and reporting as required.
- Ensure clinic and patient supply inventory is appropriate and place orders in a timely manner to avoid fees and penalties.
- Continuously seek to improve patient and quality outcomes through tracking and analyzing program and patient performance.
- Absorb and document routine and non-routine laboratory test results and communicate critical values and urgent patient needs to the responsible physician or physician extender promptly.
- Review and document monthly patient medication profiles.
- Administer medications as ordered by the physician.
- Perform on-call nursing services, nights and weekends, on a rotational basis as needed or assigned.
- Coordinate home evaluations with technical services to ensure proper equipment operation and complete required water sampling.
- Monitor and report water sample and culture results.
Qualifications/Requirements
- Current RN license in applicable state, maintained as current and in good standing.
- At least 12 months of clinical experience in providing nursing care and 3 months of experience in the specific modality for home dialysis training, except in states with stricter requirements like Texas.
- Previous experience in providing nursing care to patients on maintenance dialysis preferred.
- CPR certification required prior to patient care assignment.
- Successfully complete the Home Therapy Nurse Orientation program and maintain annual demonstration of skills and competency applicable for the modality assigned.
- Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
- Basic computer skills; proficiency in all USRC clinical applications required within 90 days of hire.
- Meet any practice requirement(s) for the applicable state.
