About the role
This role involves educating patients and caregivers on PD procedures and self-care, managing a caseload of PD patients, conducting home visits, and providing on-call support. The role also includes collaborating with interdisciplinary teams to ensure safe, high-quality outcomes.
Responsibilities
- Train patients and caregivers on PD procedures and self-care
- Manage a caseload of PD patients, including assessments and care planning
- Monitor treatment progress, conduct home visits, and provide on-call support
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes
Requirements
- RN license in the state of practice
- Minimum of 12 months of RN experience
- Valid driver’s license, insurance, and reliable transportation
- ADN required; BSN preferred
- Basic computer skills (MS Word, Outlook)
Qualifications
- No dialysis experience required
- Nephrology nursing provides a fresh lens and set of skills
- Patient education or teaching background
- Case management or telehealth experience
- CNN or CDN certification preferred
Skills
Excellent communication skills, ability to work independently, and strong organizational skills are essential.

Schedule
Flexible Monday-Friday, 8-hour shifts. Weekends and holidays off.
