About the role
The Diabetes Nurse Educator supports patients through education, intervention, and clinical coordination within a multidisciplinary care team.
Responsibilities
- Assessing patients for early signs of diabetes-related complications to enable timely intervention
- Educating patients and families on diabetes self-management, lifestyle changes, and disease prevention using evidence-based resources
- Developing and leading individual and group education programs aligned with ADA guidelines
- Monitoring educational effectiveness via patient feedback and health outcomes
- Providing instruction on safe use of medications and devices like insulin, CGMs, and pumps
- Reviewing and interpreting blood glucose and device data for care planning
- Assisting with device troubleshooting and sensor management
- Collaborating with families and the interdisciplinary team across care settings
- Supporting compliance with healthcare standards and promoting diabetes awareness through outreach
- Maintaining standards for ADA credentialing and ongoing professional development
Requirements
- Minimum 2 years healthcare experience with proficiency in RN curriculum
- Bachelor’s Degree in Nursing and current NY RN license
- CPR certification required within one week of employment
- Certified Diabetes Educator within 2 years of hire
Preferred Skills
- Experience in primary or specialty care settings
- Current driver’s license
- Additional certifications relevant to specialty areas
Work Environment
Office setting with exposure to equipment cleaning solutions, requiring mobility, occasional patient lifting, and adherence to infection control protocols. The role involves working near others in a fast-paced environment, with PPE use as appropriate. Patients served span all age groups.
