Diabetes Nurse Educator

Role: Diabetes Nurse Educator responsible for delivering diabetes self-management education following ADA and ADCES standards. Key Responsibilities Assess patient learning needs, goals, and barriers; tailor educational interventions accordingly.Lead individual and group sessions on diabetes management, including medication, nutrition, exercise, and blood glucose monitoring.Provide formal diabetes management instruction for inpatient and outpatient care.If qualified, deliver medical nutrition therapy services.Support ongoing diabetes management through follow-up and collaboration with healthcare teams.Stay current on diabetes technology, protocols, and industry developments.Document services in compliance with CMS and payer guidelines.Serve as a health resource for the community and participate in educational program development.Maintain necessary certifications, complete continuing education, and adhere to licensure requirements. Qualifications & Skills Current RN or Registered Dietitian license; CDCES certification preferred or to be obtained within 2-3 years.Degree in Nursing, Dietetics, or related field; BSN preferred for nursing, Master's in Dietetics if applicable.Minimum 2 years of patient-focused educational experience preferred.Knowledge of diabetes management, patient education, and healthcare protocols.
