Diabetes Educator RN PRN
About the role
Provides direct patient care focusing on assessment, diagnosis, planning, implementation, and evaluation within nursing standards.
Promotes patient and family-centered care, educating on illness management, medication, and post-treatment needs.
Ensures patient safety, quality, and regulatory compliance while engaging in professional development.
Collaborates with the healthcare team to develop and communicate individualized care plans, considering physical, psychosocial, emotional, cultural, and spiritual needs.
Uses critical skills to evaluate patient conditions and barriers to involvement in care.
Must work safely and maintain alertness at all times.
Qualifications & Skills
- RN licensure with a Nursing Diploma/Associate's degree; Bachelor's preferred
- No prior experience required; 2 years preferred for advanced roles
- Previous diabetes education experience is required
Key Responsibilities
- Educate patients and families on illness management, medication, and survival skills
- Develop, implement, and document personalized care plans in collaboration with the team and patients
- Assess physical, psychosocial, and emotional needs, addressing patient preferences and barriers
- Participate in safety initiatives, quality improvement, and professional growth activities
