About the role

Provides oversight for planning, implementation, and evaluation of the DSME/T program and ensures the systematic and coordinated delivery of diabetes educational services. Reports to the Wellness Manager.

Responsibilities

  • Plans, coordinates, implements, and evaluates the activities and curriculum of the Diabetes Education Program in a manner consistent with practice standards. Maintains appropriate documentation of education in the electronic medical record and statistics to maintain national program accreditation.
  • Develops and maintains relationships and partnerships with community groups, payers, and potential referral sources. Interfaces with the Volunteer Diabetes Program Accreditation Advisory Group.
  • Performs assessments of referred patients, educates and evaluates participants, and serves as a resource to patients upon program completion.
  • Oversees the diabetes education process to ensure services are provided in an individualized and fiscally feasible manner.
  • Develops and directs the implementation of an annual program evaluation plan and performance improvement activities.
  • Performs high-quality assessments of infant, pediatric, adolescent, adult, and geriatric populations. Maintains awareness of the age and literacy level of patients when documenting, planning care, and providing follow-up services.
  • Performs systematic annual reviews of the program to foster quality healthcare services and ensures all services are provided with compassion, understanding, and respect.
  • Plans and coordinates high-quality continuing education programs to ensure practice personnel receive clinical training and education.
  • Acts as a clinical consultant to practice staff to promote high-quality care.
  • Promotes quality and efficiency by staying current with trends in the field through seminars, workshops, conferences, and professional affiliations.
  • Develops positive relationships between the center and community agencies, civic groups, and professional affiliations as the center’s representative.
  • Works through civic groups and public education activities to improve community health and foster the center’s image as a healthcare leader.
  • Coordinates with Business Development in planning and executing marketing activities.
  • Performs other duties as assigned or requested.

Qualifications

  • Minimum of 5 years of clinical experience as a Registered Nurse or Registered Dietitian, with 2 years in diabetes education.
  • Certification in Diabetes Education (CDE).
  • Education in, and/or experience with, chronic diseases and disease self-management.
  • Degree from an accredited school.
  • Knowledge of chronic disease management and disease self-management educational processes.
  • Strong program management, oral and written communication, interpersonal, critical thinking, organizational, time management, and computer skills.
  • Proficient in various computer applications, including spreadsheets.
  • Licensure as a Registered Nurse or Registered Dietitian.
  • BLS Certification.
