About the role
A healthcare professional specialized in diabetes education to assess patient learning needs, develop education plans, and deliver self-management education in inpatient or outpatient settings.
Responsibilities
- Assess patient, family, and caregiver knowledge and readiness to learn about diabetes management and complications.
- Provide technical training on blood glucose monitoring, insulin administration, and pump management.
- Monitor patient responses, collaborate with healthcare team for treatment adjustments, and offer ongoing education.
- Ensure program compliance with National Standards, oversee education program operations, and maintain staff qualifications.
- Participate in quality improvement, program evaluation, and serve as a resource for staff and patients.
Qualifications
- Graduate of accredited nursing, pharmacy, or dietetics program.
- Professional licensure relevant to the role, including BLS certification and certification as a Diabetes Educator (CDE).
- Experience is not required but may be preferred.
Skills
- Strong patient education and health literacy principles.
- Effective communication, teamwork, and program management skills.
- Ability to work safely and perform duties accurately.
