About the Role

Highly innovative and fast-paced program seeking a high-energy, organized, and flexible Diabetes Care & Education Specialist. The specialist serves as an expert and resource for patients and healthcare providers in diabetes self-management assessment, education, and follow-up. The main goal is to equip patients with the knowledge and tools needed to manage their disease.

This position operates in the outpatient setting, working directly with patients one-on-one, in group classroom settings, and over the telephone. The role involves collaborating with colleagues, leadership, physicians, and hospital services to develop an effective, patient-centered program that promotes optimal patient outcomes.

Responsibilities

  • Plan, coordinate, and facilitate diabetes self-management education, training, and support.
  • Conduct thorough diabetes assessments and collaboratively identify education needs, goals, and interventions with patients.
  • Address specific nutritional concerns as outlined in each patient’s initial assessment and include them in the treatment plan.
  • Monitor and evaluate the effectiveness of interventions and goal achievement, revising as necessary.
  • Identify best practices in diabetes self-management education, training, and support.
  • Analyze and evaluate program-related data from multiple sources to ensure high-quality, benchmark program initiatives.
  • Provide diabetes self-management education, training, and support to patients within an accredited Diabetes Self-Management Education (DSME) Program.
  • Deliver patient education in both individual and group settings on a wide range of diabetes self-management topics.
  • Act as a diabetes resource for colleagues, patients, other departments, and the community.
  • Seek educational opportunities to stay current and competent in all areas of work responsibilities and meet continuing professional education requirements.
  • Prepare materials such as reports, memos, letters, procedures, instructional and marketing pieces, and assessments.
  • Participate in community diabetes committees and/or diabetes advisory committees.

Requirements

  • Registered Dietitian in the state of Florida.
  • Upon hiring, pursue education and certification as a Certified Diabetes Educator (CDE) by the end of the first year.
