Diabetes Advanced Practice Provider (NP/PA)

This role will lead technical initiatives focused on scalable systems, platform reliability, and meaningful business transformation. You will work across engineering, product, operations, and business stakeholders to translate complex requirements into practical technology solutions. Location: Remote - US based candidates only, no visa sponsorship available

Compensation: $130,000 – $170,000 annually

Responsibilities

• Conduct thorough assessments of patients via virtual consultations
• Formulate diagnoses and develop individualized diabetes treatment plans
• Collaborate with care team members to ensure holistic patient care
• Provide clinical escalation support for specialty clinic staff during business hours
• Engage in panel review discussions for advice on medical management
• Perform comprehensive assessments for quality care and patient needs
• Utilize and provide feedback on the electronic health information system

Qualifications

• Master's or Doctoral degree in nursing or physician assistant studies
• Active RN and APRN or PA licensure at time of hire, maintained while employed
• Minimum 2 years concentrated in diabetes management and education
• 5+ years outpatient clinical experience, ideally in endocrinology or diabetes settings
• Proficient in telehealth technology and electronic health records (EHR)
• Multi-state licensure required; willing to obtain more as needed
• Proficient in English; Spanish proficiency preferred
